Post-Treatment Support

We're dedicated to facilitating a compassionate and seamless transition to lower-level outpatient care through our Warm Handoff, ensuring your clients receive the ongoing support they need as they near completion of inpatient care. We're here to support both your staff and your clients through this important change, providing the aftercare and guidance your clients deserve as they transition to a lower-level of psychotherapeutic care.

Empower your clients with a Warm Handoff  to lower-levels of care

Benefits of a Warm Handoff

A warm handoff is a transition of care that involves direct communication between the outgoing and incoming providers, often with the patient present. Overall, warm handoffs contribute to a more patient-centered approach to aftercare, promoting collaboration among providers and enhancing the quality of care delivered.

Improved Communication

A warm handoff ensures that critical information about the client's condition, needs, and preferences is effectively communicated between healthcare providers. This direct exchange minimizes the risk of miscommunication and helps to clarify any questions, leading to better continuity of care.

Enhanced Client Experience

Involving your client in the handoff process fosters a sense of trust and reassurance. When clients see their providers working collaboratively and discussing their care plan together, they feel more valued and engaged in their treatment, which can lead to increased satisfaction and reduced anxiety.

Reduced Errors & Improved Outcomes

By allowing for a thorough discussion of the client's history and current status, warm handoffs can help prevent errors that may occur during transitions. This continuity helps ensure that your client receives appropriate follow-up care, leading to better health outcomes and reduced readmission rates.

Having worked at, and run several treatment centers, I know how important aftercare planning is. I also know how overwhelming it can be to find your client the right fit while still in the early stages of treatment. The best aftercare planning begins soon after a client's first day of inpatient treatment. This is where we can help.

At Affirmative Counseling Center, our team of professionals are prepared to assist you in placing your clients in our lower-levels of outpatient care, including:

  • Weekly Individual Psychotherapy Sessions (multiple if needed)

  • DBT Individual Counseling

  • DBT Skills Group Counseling

  • Substance Abuse Counseling

  • Psychiatric/Medication Management

  • Couples Counseling

  • Family Counseling

  • EMDR Therapy

We are happy to collaborate with your therapists, case/care managers, outreach specialists, etc., to assist in this transition. We meet with your client to begin a relationship that helps them sort through all our options and build a level of comfort.

Most out of network PPO insurance plans are accepted. Our internal billing experts can quickly check your client's benefits and bill directly, so no superbills are needed.

Looking forward to working with you!
